Spring is always the busiest season for plastic surgeons, as people want to get ready for the summer. As winter blues are giving way to "spring is in the air", thoughts are projected to beach scenes and the wearing of a bathing suit. Concerns regarding the stomach pouch, saddle bags, back folds, knock knees, and chin pouches all seem to creep up. Most women feel that somehow these were not as bad before the winter. And of course, despite intensive workouts they just don't seem to go away. Well, good news! Most of these problems can be handled with liposuction. Liposuction has undergone dramatic changes since its introduction in the early 1980's. Cannulas, which are the tubes through which the fat is removed have become much thinner allowing for smoother contours.
The use of tumescent solution, which contains physiologic salt water and very dilute epinephrine is the mainstay of the operation. It dramatically reduces bleeding, thereby minimizes bruising and allows for the safe removal of much larger amounts of fat at one session.
There have been innovations in the past few years which you may have heard about. One is Ultrasound Assisted Liposuction (UAL). This procedure uses ultrasound to assist in then removal of fat believing this may facilitate the procedure and further minimize irregularities. After carefully studying and reviewing this technique, I feel that UAL has no real advantages over the tumescent technique. It actually has some disadvantages. These include a larger incision, the possibility of skin burns, and it is more costly.
Recent reviews in the plastic surgery literature have corroborated my point of view. I feel that instead, the use of postoperative massage and ultrasound treatments accelerate the healing process by helping to reduce swelling and minimize postoperative hardness. I have used this therapy since 1985, and it has proven to be extremely beneficial. Most of all, patients love the treatments.

You may have also heard about Power Assisted Liposuction. This is nothing more than the use of power for the to and fro movements of the cannula. This can ease the surgeon's fatigue but has no real impact on the results. I believe that surgeons with experience do not need the power of the machine, and may actually have a better feel for the operation without the outside power.
I believe that ultimately it is the plastic surgeon's experience and sense of aesthetics that will ultimately determine the results of the operation.
